You both must go to your county clerk in person with ID. You apply for the license online but you still need to go to the clerk to pick it up. You both need to go with ID to sign the application.
The marriage license needs to be returned to the clerk after you are married. Legally, you have 30 days to return the marriage license to the clerk. You do not have to go together. Anyone can return a marriage license. You can return a marriage license by mail.
No witnesses are required to be legally married in Indiana.

Your name does not change automatically when you get married. You can change your name with social security AFTER the wedding.
